Thank you very much for purchasing one of our Health and Wellness Tests.
1. Please read and follow the steps below to send and receive your Health and Wellness Test.
2. Go to the following link to download and complete the health form.
Take a small cutting of your hair (approx. 1cm in length and at least 10 hairs). This can be hair from your head, arms or legs. If you do not have enough hair you can include some clean cuttings of nails. For hygiene reasons, place the hair and/or nails in a clean tissue, cling film, small envelope, no staples and attach to form please.
Ignore the payment details on the form if you have already paid.
3. New Customers/Patients
Please follow the instructions below ensuring that you have clearly specified at the bottom of your form which test you have chosen eg Vitamin/Wheat/Dairy Test.
If you are an existing customer/patient please send in a fresh hair sample clearly marked with your patient ID which is on your past results letter. If you have not got this we should still be able to locate your details in our medical scanner but will need your hair sample to do the test. Please clearly mark at the top of your form or letter with hair sample which test this is for.
If this is a subsequent test please send your unique Patient number which can be found on the heading of your results.

To avoid delays, please ensure you follow the posting guidelines below:
Attach the correct postage to your envelope. Often this will require a ‘large letter’ stamp rather than a standard first or second class stamp. We are charged the difference in postage charges plus a standard £1 fee by Royal Mail to collect letters with insufficient postage.
Do not send your letter by Recorded Delivery. This slows down the process of your test
We must receive your forms and hair sample before the final redemption date. Any forms received after this date may not be processed
4. We will send you email confirmation once received but if you do not hear from us within 5 days of your sending your sample and form you must contact us as we cannot offer you this test at a later stage due to mail delivery failures. It is imperative that you check that we have received it.
We will email your results back to you within 21 working days. Please check your junk box for results as it has been known that some results have been found there. If you do not receive your results within 21 days of receiving our email confirming receipt of your health form and hair sample you must let us know as we cannot process this after the redemption deadline has passed.
